What book is it actually???

I checked amazon-dot-com immediately following Ed's response and there were two
versions:

Lineman and Cableman's Handbook by Thomas Shoemaker
Used from $50.00

Lineman and Cableman's Handbook by Edwin Kurtz
Used from $15.00
