Quite a list of the candidates...But I think we can exclude point #8.In that case the energy is released so fast that racing sparks don't have time to develop I think even though critical field might be sufficient.Anycase I never heard of racing sparks problem being reported then.

I don't think any progress has been made to narrow down the
list since then. Probably it is not too great an issue,
reduction of coupling always (I think) cures the problem.
It's just that, to our embarrassment, we don't know why.
